Personal Health Record Data Transfer Between Health Plans

America's Health Insurance Plans and the Blue Cross and Blue Shield Association


AHIP and the BCBSA developed a model health plan-based personal health record (PHR) and standards enabling the information to be portable between health plans giving consumers the ability to take information with them if they change health plans. The standardized solution is an XML document encapsulated inside an X12 formatted transaction set and described using the familiar Washington Publishing Company style. It is AVAILABLE FOR FREE from this web site. The technical document includes the technical standards, data dictionary, and operating rules for plan-to-plan transfer of key PHR data.
